Blue Streaks Athletics Registration Forms Now Available for 2016-2017

Student Athletes need to be in attendance at the first day of practice to try-out for a team. Unavoidable conflicts must be dealt with in advance through the Head Coach. In accordance with OHSAA rules, in order to participate in tryouts, all student athletes must have the following forms on file with Head Athletic Trainer, Maureen Sizemore:

  1. Proof of Insurance
  2. Athletic Transportation Policy
  3. Emergency Medical Authorization Form
  4. Handbook Acknowledgment
  5. Ohio Department of Health Concussion Education Form
  6. OHSAA Authorization

The Pre-Participation Physical is the only form that must to be turned in hard copy with a Physicians's signature to the Athletic Sports Medicine Department, 
prior to the first day of the season.

2016-2017 Ohio High School Athletic Association Preparticipation Physical Evaluation / OHSAA Authorization Form (6 pages).  A student athlete's physical is good for one calendar year.
